      NOTE: If you are unable to commit to any one of the requirements, please do NOT complete this application—your application will NOT be reviewed if you select “no” to any requirement.

      Your child has elected to submit an application for the NCNW BHCP Girl Conference (GirlCon). 

      In order for your child to participate, NCNW requires your consent––please carefully read, complete, AND sign this Permission Slip section of their application.

      In connection with and consideration of my Student’s participation in the BHCP GirlCon,  I, on behalf of my Student, myself, our my heir(s), personal representative(s), guardians, and assign(s), hereby represent and agree as follows:
       
      • ACTIVITIES: Parent consents to Student participating in all GirlCon activities. 
      • LOGISTICS:  
        • DATE: GirlCon will take place Saturday, April 5, 2025, 8:30AM-4PM.
        • LOCATION: GirlCon will take place in the D.C. Metro Area.
        • TRANSPORTATION: Transportation to and from GirlCon is the responsibility of the Parent—NCNW will not provide transportation.
      • MEDICAL: 
        • ASSISTANCE: Should my child require emergency medical treatment, first aid, or transportation to a hospital or medical facility as a result of illness or injury, I consent to any such treatment.
        • RECORD: My child does not suffer from any undisclosed disease, injury or health related condition that would preclude or restrict his/her participation in this program or related activities or otherwise render his/her participation dangerous.
      • STUDENT RECORDS: I authorize the release of my child’s grades and progress reports for a period of one year, to be used only to measure program impact and assessment of participant’s progress.  
      • LEGAL: 
        • HOLD HARMLESS: I agree to indemnify and hold harmless The National Council of Negro Women, Inc., its officers, directors and employees and volunteers from all Claims arising out of, related to, or in connection with GirlCon that are caused by my or my child's negligent or intentionally tortuous acts and/or omissions. Hereby agree to hold NCNW, its officers, directors and affiliates harmless for injury or damages, except as caused by the negligence of NCNW.
        • INTELLECTUAL PROPERTY: All materials, articles, research, or other intellectual property that Student may create or contribute to during, or as a requirement of BHCP, is collectively referred to as “IP”. 
          • USE: NCNW agrees that the IP described herein shall not be used for commercial purposes. NCNW has the right to publish, distribute, edit, and modify the IP at its sole discretion. Rising Changemaker further understands and agrees that these materials may be kept on file and used by NCNW for potential future purposes and further agrees to release NCNW from any liability arising from or in connection with the use, publication, or dissemination of such materials.
          • OWNERSHIP: Parent agrees IP shall become and remain the sole property of NCNW, and Parent nor Student shall have no right or title to such items.
          • COMPENSATION: Parent and Student agrees that NCNW does not owe them any compensation for the acts or materials that Rising Changemaker has consented to in this Agreement. 
        • LEGAL RIGHTS: I, the undersigned, certify that I am the parent or legal guardian of the child (named above) and that I have the right to make decisions for the Child that affects their well being. 
        • LIABILITY WAIVER: Parent aware of risks associated with Student’s participation, including death or injury and property damage or loss. Should any such damage or loss occur, Parent agrees to hold NCNW harmless—meaning, Parent, Student, nor their assigns, conservator, guardian, heirs, or personal representative will seek to hold NCNW civilly or criminally liable.  
        • NAME, IMAGE, AND LIKENESS (NIL): Parent authorizes NCNW to film and photograph Student and consents to the use of Student’s likeness and image in all advertising, educational materials, marketing, media, news publications, research, and web materials. Parent understands and agrees that such materials, including all negatives, positives, digital images, and prints shall become and remain the sole property of NCNW and Parent nor Student shall have no right or title to such items.
        • SUCCESSORS: ‍The provisions of this Agreement shall be binding on and inure for the benefit of the Parties and their respective successors and assigns and legal representatives.
        • APPLICABLE LAW: ‍The interpretation of this Agreement or any part of it shall be governed by and construed in accordance with the laws of the District of Columbia and shall be subject to the exclusive jurisdiction of that Federal District.
